I understand that reflexology is not a substitute for medical care and that my reflexologist does not diagnose, prescribe, or treat medical conditions.*

If I experience any discomfort during my session, I agree to inform my reflexologist immediately so that she is aware and can adjust the session to my comfort level.*

I agree to inform my reflexologist of any medical conditions, allergies, injuries, and/or possibility of pregnancy.*

I understand that I may discontinue a session at any time; however, refunds may only be provided when canceling more than 24 hours in advance.*

I agree to inform my reflexologist of any change in my health. *

I understand that my reflexologist reserves the right to refuse service for any reason.*

By signing this release, I hereby waive and release my reflexologist from all liability, past, present, and future, relating to reflexology.*
